New Stouffville Covid Tracing Form

All patrons are now required to complete a Town of Whitchurch-Stouffville Screening & Contact Tracing form prior to entering any Town facility.

One form per family (for parent and child) is acceptable. This process will be in addition to any procedures you have implemented as an organization.
